What are you doing during those four hours you say you're not in group? Standing in the hall or outside smoking? If it's that unproductive, I'd wage a complaint to the PHP director or higher up. My PHP kept us in some kind of group or class of sorts 50 out of 60 mins other than time with the pdoc, nurse, or 40 minute lunch. Mine was expensive. I expected my money's worth.

Please be sure not to rush back to work prematurely. That often but leads to relapse, which makes the situation worse.
