If your wife has to take your pills so you don't OD you need inpatient. I am so sorry you are feeling so bad. Is there a place you have been before that you would prefer--maybe your psychiatrist can call ahead for you. Otherwise the ER may be the way to go. If you are that unsafe with yourself at the moment you probably should not be alone until you can get help.
