Since getting a new pdoc can be hard, and you may find yourself on a waiting list 3, 4, 6 months long, reschedule with the old doc at your next appointment. Call an make an appointment with a new pdoc but keep seeing the old doc until your appointment comes around. After you see the new doc, your care will technically be transferred, though of course, you might have to or want to sign medical release forms.

But don't cold turkey stop the old pdoc until you have the appointment scheduled with the new pdoc and know how long you will have to wait. It's worse to have no pdoc at all.

I wouldn't mention changing doctors to him until you have an appointment set up with a new doc.
