I've never seen them go up to 25 lbs. My 22 lbs is about as high as I've seen. The place I bought mine has gone out of business. I can't imagine any heavier than mine to deal with making the bed etc. I'd say 20 lbs or so is probably good.

Mine is Minkee dots. It is pretty warm but I'm the kind of person who sleeps under 3 comforters and 2 quilts and maybe a blanket or two in the winter and a blanket and comforter in the summer (plus weighted blanket for both). I found the soft fabric was calming and I rubbed the dots between my fingers until they worn down to flat.

Mine came sized like bedding sizes.

I have no idea where to get one that is covered by insurance. I wish I did.
