To reply to your first part, as I see you're in Canada--in the US, many therapists don't take insurance--you just pay them a direct fee, then can sometimes get reimbursed for part of it by insurance (that's how my T, ex-T, and ex-MC operated). They're considered "out of network." It keeps them from having to deal with insurance paperwork and restrictions that can be placed on them by insurance companies (like treatment limits). However, it can also make it considerably more expensive for clients, particularly if they don't have out-of-network benefits in their insurance (my insurance does, reimbursing 60%).
