I try to thank at the time; when I learn something new, or they do something I hadn't noticed before, when they "give" me something I didn't have. I think the session itself is usually just part of the whole working relationship for which I pay so if it's pretty much just a "normal" session, I wouldn't think to say "thank you" on the way out.
