If anyone can help me out I’d be so grateful. I know I can use google but I figured asking here would be better with more accurate results.

Here’s my problem: I finally said enough is enough and I made myself an appointment a new psychiatrist. This is I think the 4th one I have started out new with. The thing is with my very first, long interview visit, I want to bring in all my medications and a very long note of everything about me and what I go through.

The weird problem is now that I need to know certain small things I deal with or something I go through or something that happens to me, I can’t think of it!

I have basic things written down and such so far.

What I’m basically asking is if you can list your symptoms or what you go through or deal with during:
Mania
Depression
Mixed
Hallucinations

I want to read through and see if I relate so I can put it in my note. I just want everything with my new pdoc to go well.
