I'm having a very weird symptom and I don't have any idea what's happening.

When I'm sleeping with someone I love or admire too much I'm suffering premature ejaculation which is really embarrassing and destroyed my relationship before twice with ladies that I loved so much however if I had casual sex with any lady that I don't have actual love and admiration feelings towards her I don't suffer any premature ejaculation and I don't even feel internally excited while I'm having sex with her.

I checked with urology doctors and they confirmed that I'm not suffering any physical illness and they even consulted to try some medications for premature ejaculation but unfortunately with no effect. even if I was under medication once I start intercourse with a lady I love I suffering the same issue and the opposite extreme with any casual intercourse without any medications.
