She'll probably understand if you explain it to her. Was the Spongebob shirt different from what you usually wear to therapy? Maybe she thought it was more casual or something. Careless dressing (dirty or inappropriate clothing) can be a sign of depression so she might just be checking on that. I know if I showed up to therapy in sweats my T would ask about it, not because he judges my pants but because it's not normal for me.
