Meds and therapy have their place, but is there anyone in your world who can assist you with the
practical aspects of the situation you are facing? In the US
2-1-1 theoretically could put you in contact with agencies that can help.

My dog
mastered the "fetch" command. He would communicate he wanted something, and I would fetch it.
