This is how we were taught to document. The funny thing though is I was taught to document every detail and others interpret the same thing to mean document very little. With therapy it's different; they document little so they can't be forced to share details of therapy in court but otherwise in medicine they teach paranoia in documentation. 8 years after I left I still wonder sometimes if an error in my documentation could ever haunt me. It can't; medical records are purged after 7 years but it was deeply instilled in me to be cautious, beyond cautious.
