I'm really bad about calling. My pdoc reminds me at every office visit that I'm supposed to call the minute I feel the least bit unwell, because I tend to get very bad very quickly (in both directions). I haven't had much in the way of mood swings in a while now so it's a moot point at this moment in time, but of course it can all change in the blink of an eye so I'm always wary. But knowing that I'm reluctant to call when I'm in trouble, my son has my written permission to intervene if necessary.
