I would suggest just simply saying "Doctors Appointment" they can't argue with that, since you are going to see a doctor. if they ask for a doctors note, or some sort of proof, give them the name/number of your doctor. You don't have to say specifically what you're going for. Simply because it's none of their business.

I know where I work, when i want a day off, a request a little in advance.
