If I was in that situation, I would get a new doctor. You can request to see your information to give to your new doctor so they know what all she has you on. She sounds like she doesn't care to take better notes to remember what you talked about. I get that she sees a lot of people, but that's why notes are super important. If she doesn't listen to you then she can easily prescribe you something that will cause more issues than you staying sleepy.
