They will send your therapist and/or any doctors they want to hear from forms to fill out. You don't have to worry about that. The forms will ask a lot of questions about how you are able to function and how your diagnosis affects your life, much like the ones you fill out. My therapist fills his out with me so we know we are consistent (yet honest).
