I've had something similar happen me at the Social Security Office. A caseworker was denying any decision I had made and was ignoring anything I'd put in writing. She kept acting as though she was not getting my letters .I finally wrote a letter, now "certified," to both her and to her supervisor. Again, absolutely no response from either party. The supervisor did advance my claim; however, she'd never told me so.

If you need the paperwork signed, and there is some time, you might try again with your pdoc's secretary. It is quite likely you will have to continue working with her while working with your new pdoc? Maybe try to become friendly and try again with the paperwork? If you'll never trust her and you need your pdoc's signature, mail the paperwork directly to your pdoc. When you meet with your pdoc, tell him of your experience.

Is it possible for your T to sign the paperwork? if so, go that route.. If not, why show him/her the details? Maybe it helps with validation?

If your t cannot sign the paperwork, try again with you pdoc and/or his secretary. Also recount your experience with your T.
