Definitely permanent or current:
Bipolar type 1 disorder, hypothyroidism, mild kidney damage, mitral valve prolapse (heart), cervical polyps, ovarian cysts (unless they've since disappeared on their own), and genital herpes (Oh well!). I also have some anxiety challenges, but no formal diagnosis for them, though once a hospital assigned the GAD code along with my bp. All but the bipolar disorder are managed very well. I do have high cholesterol and triglycerides, but I hope to someday change that.

Not definitive
Epilepsy (simple partial seizures) and a history of alcohol abuse (I have the history, but it doesn't seem to be pure alcoholism. It's not currently a problem.) If I do have epilepsy, it's managed well by my Tegretol. I do not want to ever stop an anticonvulsant moodstabilizer.

I've had other mental health and physical conditions, but they are seemingly resolved. I do have issues with sinusitis at times during the year. I take a medication for tachycardia, but it is well controlled. I'm not sure how much of that was anxiety, my mitral valve prolapse, or an old medication.
