My husband wants to delay his next eye appointment in Philly. He feels a bit wary about going, since he usually needs an injection in his eye. That place is usually a mad house with crowded waiting rooms. They sent him an email saying patients would check in and be asked to wait in their cars in the parking garage/lot. I have zero clue how they make that work! Since it's in downtown Philly, many patients likely go there by train/bus or taxi. Where do they wait? Many are elderly. The whole ordeal usually takes 2 hours as it is. Who knows, but I am sure many are delaying appointments. His eye specialist had a heart transplant. If I were that doctor, I really would avoid seeing patients.

That's strange. I agree it makes no sense to have people wait in their cars if most people do not have/use cars.
What they should be doing is limiting the number of appointments and removing chairs so that people are sitting 6 feet apart. For example, 2-3 appointments max at any one time. This way, only 2-3 people (plus a guest) can sit in the waiting room at any given time. That, to me, would make more sense. That's what the local Toyota dealerships around here are doing. Anyone else who comes early has to hang out outside. They only let people sit inside if their car is currently in for service.
