What is UTO? Unpaid time off?
In my opinion, it's worth asking for accommodations because you're on thin ice, as you say.
My main concern is that your employer may think you're making up excuses to get extra time off. Although it's possible they'll be understanding, it's likely they don't fully understand bipolar (or mental illness in general). They may also get upset if you didn't list that you have a disability upon hiring since bipolar is technically considered one. Not sure if you declared it or not, though.
I think you can try to work something out.
