If I were you I would discuss a low dose antipsychotic to reduce your chronic panic attacks.
From my experience with benzos, and considering the frequency and intensity of your panic attacks, I agree with you that you're looking at the problem of tolerance (needing more Ativan to have the same effect).
Another problem with benzos is that when you're taking them prn you're putting yourself on a roller coaster ride of emotional ups and downs.
