Hi Angie, maybe I can help. If you would like to either post the letter or pm it to me, I would happily go so far as to completely reconstruct it for you if it should need, to be the letter you want. Your concern is plainly valid, and the documentation of it is imporrant. It sounds to me like a documentation of incident letter to me. But you could explain it all.
