LT, if it makes you feel better, you're still paying almost exactly Liz's normal rate and even slightly more than what my insurance says she can charge me. (She has a PhD, which I think Dr. T has too?) The cost of living might be somewhat lower here than where you are, but I still don't think $133 is unfair.
