Crook32...my T has me fill out those surveys as well. They are only 4 questions and you click on a line to show how you feel about each topic. Each survey is different. I'm not sure if this is the same system used by your T but for me the first survey is about how I feel based on the prior week and the second survey is rating the therapist during that session. It takes about 15 seconds.
