Is this even worth going through the trouble? I’m regretting it again. They are super excited to have me but I’m nervous about telling them I now need an extra week. I mean, I’ll still be there for the Christmas rush.
I’m just not exactly sure how to handle this. Also can they ask what type of surgery I’m having? Is that a legal question to ask?

As long as you told them at some point, I don't see a problem in asking for that extra week off.
I don't think they can fire you for that. I am not sure as to the legality of it, but I am pretty sure they can't ask you what kind of surgery you're having. If they do, you could probably just make something up if you're not comfortable telling them.
