Right now I'm seeing my pdoc every two months, if I'm doing bad it's more frequently like once a month or every couple weeks. I talk to my therapist every 3-4 weeks depending on her schedule, and more frequently if I'm not doing well. My care manager I am able to text her anytime during the weekdays if I'm really struggling but I probably only text her once or twice a month to have her help me schedule rides for my appointments and ask if I need help getting connected to any resources. This past year we've only met like 2 or 3 times in person due to covid and only to do my treatment plan and then for her to give me the paperwork for applying to a vocational rehabilitation program.
I live in supportive housing so there's also an apartment case manager who works here in the office, and she can get in touch with my doctor, therapist, and care manager if necessary. She'll contact them if she's concerned about me like if it seems like I'm isolating or something like she did recently. I can talk to her anytime too.
So I'm lucky, I have a lot of support
