Being a child who is not free to leave the care of people who are neglecting you would indeed be a deeply distressing or disturbing experience. Kids have a lot of needs and are generally pretty helpless. The very process of being neglected often distorts the kid's worldview so that they don't clearly understand how distressing it is. That can be a survival mechanism and can make it difficult to dig out from the aftereffects.
