Many psych medications can cause withdrawal effects. Some may cause symptoms after missing just one dose but it all depends on the medication. I think it's very likely missing your Cymbalta may have triggered your attack. Check with your doctor or pharmacist.

I take Effexor which is similar to Cymbalta and I have experienced horrible withdrawal symptoms after missing just one dose.
