In my area, most private practice psychiatrists do not accept insurance. They give patients a superbill and patients then bill their own insurance IFF they have private insurance with out-of-network coverage. But it does not work this way with Medicare. So patients with Medicare are out of luck with most psychiatrists who are in private practice.
