One key element in exercise is increased oxygen intake. You can accomplish this without physical exercise by deep breathing exercises, which have the same aerobic effect as exercise.

For those in wheelchairs etc, and diabetics, it has been shown that doing arm exercises vigorously, increases circulation enough to positively affect the feet too!
