Anyone have something similar to this going on or experienced in the past? Like a trauma-based hallucination (not really a flashback except for certain aspects) tied to something else going on (in my case an eating disorder/body dysmorphia)?

To be specific, a lot this past summer I heard a voice, always at night when the street lights + my landlord/maintenance still not putting up my curtain rod (after a month of "yes, we can do it, I'll let you know when the maintenance guy has time.") = similar to night time psych ward at night lighting. Anyway the voice is the same as a male night-shift nurse at probably one of my worst psych unit experiences always commenting on my body and what I did that day mostly around food, but sometimes self-harm stuff (while on the unit I thought he was narrating my every action when it was dark). I thought that nurse was legitimately out there for a solid two months though.

Today another "I can't tell if this was hallucination, flashback, or reality," moment happened when I was collecting rocks by a homeless encampment and swear for at least five minutes thought a woman was just raped in earshot and walked by crying with a man that looked/acted really guilty. Another thing happened before my last hospitalization similar where I thought someone was being attacked on the street and I thought I should call 911 but realized more likely than not it was a hallucination (I learned while in the hospital a woman got stabbed on my street that night so felt shytty for being too out of it to know the difference between real and not real).

How do you reality test something like that? I don't want to ask people if they just got assaulted because if they didn't I come across as crazy while probably believing they're in denial/lying to me to pretend it didn't happen because I know that's what I did for too freaking long. But also want to help someone if they are being/were just assaulted in some way.
