I watched a Charlie Rose program (you probably don't know Charlie Rose) this morning about Nelson Mandela... at one point the Queen called him to congratulate him on his birthday (I'm not sure it was his most recent birthday) and Mandela said "Thank you, Elizabeth." One of his advisors told him that you don't call the Queen "Elizabeth" but of course he did and wasn't apologetic about it...
