IEP = Individual(ized) Education Plan.

Basically it helps you plan out your education and outlines your disability issues for the school and your teachers and stuff.

Have you gone to see a psychiatrist or psychologist about this at all? I'm sure they'd be more able to help you, as none of us here is really qualified to give you a diagnosis, especially over the internet.
