Actually, I have an image of my chart with Key Topics (Maybe possible Dx's) listed. Then she tick marks when I mention something consistent with the Dx.... or that is related to a key issue. Maybe... she has codes for different repsonses. H= hostile, A= Agitated, W= withholding, T= Clear trigger--hit this topic again when you at to see squirming. LOL
