I am sorry that you are having a hard time, this is not really a place to be diagnosed. Have you talked to your parents about seeing someone? That's the only way that you can have a good diagnosis is to see a professional. I agree that it sounds like things are triggering you so it may not be diet but also keep in mind that if you are on a lot of sugar sometimes it takes a while to catch up to you. When I was a teenager I had horrid eating habits and it took it's physical effects, I was tested for everything under the sun, the doctor just could not figure out what was wrong, so that is a possibility.
For now I recommend maybe looking up some grounding techniques, you find that helpful and like I said you really need to be seen by a professional to be correctly diagnosed, usually they like to rule out all the physical conditions that could cause what your experiencing before they can give you any of the answers that you are seeking. The sooner you are treated the better.
