Maybe you could start by telling them that "you've reached your breaking point", I mean I think it gets the point across that you are serious about this situation. The note idea is also a good idea, I guess it is just what you are more comfortable with. I have found that the anticipation of somethings are worse then actually doing them, sometimes it's just better to do it instead of thinking about it or that is how it is for me.
