Dustin, all you need is the information from the card so your T can bill the insurance company for her services. You can make a copy of the card and she can use that because all the information is there. Or, you can copy that information: the Group number, the Plan ID number or plan name, and the phone number for providers to call.
Maybe you could get this for her?

Perhaps there was some miscommunication around the insurance that you could clarify with her. Maybe she was expecting to have the insurance information provided to her at some point even though it wasn't available initially. It's something for you two to talk about and work out.
