It would be really hard to write down everything that you say. Therapists have to keep notes, but they are more of a quick summary. Generally, less is better in the notes - like just enough so that they can remember what they need to remember.
They don't report things like feelings, like hating someone. Like CSC and Christina said, reporting is only if there is reason to believe that action is necessary to keep you or someone else safe. Such as abuse of a child or vulnerable person, or serious plans to hurt yourself or someone else.
