AvPD is tough. Partly because the mental health industry has no clue how to effectively treat it.
For me, I started to break out of it by developing something that I was good at. Fortunately, it led me into a career that I've had good success with. I had some good mentoring, worked hard, and have a lot of determination.
So my advice is to find something you would like to do, that you consider worthwhile, and put as much energy and effort into improving. Take it and run with it. It doesn't matter what it is at first as long as you get joy/satisfaction from it. You'll feel much better once you've accomplished something you consider worthy.
