The website Timgt5 posted does; you usually don't get to the agencies until after the test/form filling out. They just use their forms instead of your own resume/references, etc. you don't turn in anything of "yours" just their stuff. It's not too intimidating, just like applying to school or something. I kind of like it because it's all standardized so you don't have to "think".
