Doing drugs, alcohol, I'm clean and sober.

How come the psychiatrist still wants to put active substance dependence? I'm clean and have no intention on going back to using again. There is no benefit to me using anymore, it was just plain and pure self-destruction. I would smoke cigarettes knowing damn well they were killing me and all the risks I just really didn't give a ****. I came to a realization after spending a long time suffering from severe pain, that I wanted to end my suffering and dying was not the easy way out I always imagined it would be. So I quit doing drugs, started going back to talk therapy and find that I'm still suffering. Healing from the wounds of the past, causes me a lot of suffering in the process. But I'm noticing the suffering is becoming less with time and practice.
