This would be a great plan to go over with a therapist and take a good long time thinking about and filling out. Thanks for posting.

A very good worksheet! I have a book called the Bipolar Workbook and it has a lot of these similar worksheets in it - I can really recommend it.
I don't like the support page - I am not sure who I can put down that I can count on. My BF is still very wet behind the ears when it comes to this. My parents don't know about my dx.

I'd love to chat to my T about when it is time to hand over my responsibilities to someone else... I battle with making that decision

Current Meds: Epitec (Lamotrigine) 300mg, Solian 50mg, Seroquel 25mg PRN, Metformin 500mg, Klonopin prn
Thanks for this!

Thanks for the information. I think I'll print it off and discuss it with my T at our next session (if I can remember until then). Perhaps I should write it in my planner...now.

I've thought about doing one a number of times, but then I start and it all seems like to much hassle, I'm not a particularly organised kind of person.... my hubby wants me to always carry a list of my current meds but its a couple of months out of date at the moment.... I'm not a very good patient!!!!
