I don't know off hand, but maybe you can get an inexpensive one. See what a pharmacy might have to offer. Also, you can write a note and put it in your wallet.

All have the necklaces engraved with information on a dog tag. The same machine you get the pet tags that say "my name is ******, my address is ******** please call my owner MM at *******" just in-case your pets get lost. They are like $6 and you can put a website address on it with all your medical info.
