Are they still considered delusions if you know they aren’t plausible. Like I know what I’m thinking or believing isn’t real but I start to get anxious about them even though I know it’s not real. I’m not like breaking from reality because I can tell real from not real so are these delusions?

Delusion, an idiosyncratic belief or impression that is firmly maintained despite being contradicted by what is generally accepted as reality or rational argument

I have this too. My pdoc says it's not a delusion even if you know it's not real, even it it bothers you.

As already answered, they are not delusions if they are not your actual beliefs. I'm not sure but it sounds like what you have are intrusive thoughts since your thoughts are disturbing but not delusional. I would ask your pdoc. Treatments for intrusive thoughts are available.
