I have had a lot of success preparing a psych packet for docs and ts.
I start with a table (chart) bio timeline that shows dates, important life events, psych events, meds and results, and physical diseases. The timeline is very illuminating and shows some cause and effect aspects.
Then I expand it into a paragraph version which gives more information. I put current status info at the top so no one missed that info. Current status, symptoms, triggers at the top of my history in paragraphs.
I put family medical and psych history after the timeline, names of doctors.
THIS APPROACH SAVES TIME AND GIVES A LOT OF INSIGHT. It also gives you some instant credibility with busy professionals. Hugs!

It sounds like a great packet you put together! I'm sure it is very helpful for your psychiatrist and therapist. I have something similar. Without it it's difficult to remember some parts of my history.
