I am a 24 year old male (65 kg, 5'11) with daily mood swings. Usually, I wake up feeling good and energetic but around lunchtime I start feeling depressed, irritable, tired (yawning) and have difficulty concentrating/remembering things. These symptoms occur each day without exception and usually dissipate by dinner time.

Medication history:

  • Prednisolone several times during adolescence for recurrent episodes of minimal change nephrotic syndrome (kidney condition), which is now in remission.

  • (Ro)Accutane for recalcitrant acne in 2010 for 6 months. Cured my acne but I developed neck pain because of inactivity. Neck pain is now minimal after swimming 2-3 times per week but mood symptoms still present.

  • I also take 1-2 paracetamol or ibuprofen tablets each night to prevent headaches (referred from the neck) that I often get on awakening.

  • I have tried the following 3 antidepressants for my symptoms: Zoloft (6 week trial, didn't help and gave me orgasm issues), Avanza (3 week trial, reduced symptoms but gave me bad joint pain and weight gain so I ceased use) and Luvox (tried for ~10 days but I think I was mildly allergic to it as it made me itchy, so stopped use and itchiness resolved).

I have my first appointment with a psychiatrist in 2 weeks time, but do you think I have depression or perhaps bipolar disorder? My doctor gave me a prescription for the antidepressant cipramil - is it worth trying?

It is good that you have an appt with a psychiatrist in a couple weeks that should help with getting any medication you made need. As far as trying cipramil i assume your Dr knows you are going to see a psychiatrist and you have discussed trying a new med while you are waiting.

Ask your doctor to check your Zinc levels as this can be a cause of acne and can affect your mood. I know your acne remitted with medication but the underlying cause is still not known. You might try the cipramil just to see what it does for the next two weeks until you see your psychiatrist.

I feel the same way, every day. I have add, gad, and depression, but i went to a family Dr. And got diagnosed. I think i need to see a real psychiatrist and really get tested.

That's really good of you to take steps to getting better, best of luck to you to:-)
