Why do they who are supposed to help IRL (mh services) fail so abysmally.

It's easy to blame funding:
The Guardian, 11 June 2016 - ‘This isn’t acceptable’: outcry at state of NHS mental health care funding
But I suspect it may also be a "cultural" problem, one found in a number (many?) of health services bureaucracies in various countries.
