I fall into so many psycho-social traps even when I “behave impeccably” or as near to that as a fuzzy bear or most people could... no wonder I suppose some people (mostly irl) have “given up on me”

How to get the balance “right” between being authentic and ??? I don’t know. It is hard being “sensitive”..... that is unlikely to change, being sensitive, however much I might want it to. And then I get close to one of the triggering topics for me, and why I can’t be more supportive here..

Certain "simple" treatments for depression, the "first-line" treatments in most medical systems, don't work for you. Fact. And you are strong enough not to tell the doctors what they expect to hear about "improvement". Telling the truth is a virtue. It may not be convenient, but it's a virtue.

What are we to do when caretakers are limited by official treatment protocols? What are we to do when they've got no idea how to help and refuse to admit it?

I don't have an answer.
Self-care? Self-palliative-care?

You are strong dear Fuzzybear. It is not easy in our forest to refuse their no.1 treatment option. Rohag is right it is honest.

We are all AS IS and cannot help being triggered by our emotions. I think for me personally the event/situation that causes the unwanted feeling can be avoided to a degree. Big and small life event sometimes force their way into my self enforced prison.

I am sorry that you are triggered. I am sending much love and respect to your cub and snake.
