My sister in law is having delusions that are causing her to do bad things to herself. She has attempted suicide before. What can be done to help her. I fear she might hurt herself or her children.

@Roman hill welcome to MSF My Support Forums. I am sorry your sister is having challenges.
The problem in the US is the HIPAA laws protect people and also make it more difficult to get someone hospitalized. The other problem is if you do intervene in the methods I mention below, your sister may be very angry at you and you are also interfering in her life and that of her children which may have unexpected consequences. That is why talking to a professional would be advisable. Some people call the police. The problem with that is often the person just puts on a show for the police and if they find out you called they may unload on you. If they do take them in for observation they can be out again in 72 hours or less. Who will take care of the kids if that happens? Other people have reported people to DSS Department of Social Services. Often the employees can be overwhelmed with the caseload and they may end up taking the children out of their home and put in foster homes. This really might be more traumatic for them than seeing their mother in a bad place. I really do not know what to do. That is why I recommend you contact a professional in your locale and see what they think. What I think would help you if you are able to get a therapist, you may be able to work through your feelings and that therapist could possibly advise you what to do. Can you take her kids for a weekend and give them a respite. The downside potential is that you may find yourself caring for them more and more or even fulltime. That is a huge weight on you and nothing to act on without serious consideration of how that will affect your homelife and your emotional stability.
