Have your doctor write a letter to the company outlining the necessity for treatment. That's where you need to start. Have you done that yet?

I agree with farmergirl - start with getting some support from your doctor. If that doesn't help, and this is an employer-sponsored health plan, get your benefits coordinator involved. They often have dedicated contacts at the insurance company and can work with them to get things resolved. Employers choose the health plans they want to offer their employees. If they have enough complaints and unresolved issues, they might decide to switch insurance companies and the insurers know this. Insurers like to keep the benefits people happy.

Make sure your record with whom you have every conversation with when you contact your insurance company. Write down when the call occurs, who you talk with, what they say, and ask for a confirmation number for the call (some provide this). Also make sure any correspondence you send is sent registered (or certified ?) mail so that they have to sign for it to prove that they get anything you send. Otherwise they may say in the future that they never received something you sent.

Not everything is covered. I'd make a list of what your insurance company does cover and see what you can do with that. If you want to try other, uncovered treatments I'd start a little fund and treatment plan to see what you can learn/if you can help yourself. Then I'd write a book
