Hi there everyone.
Well after having a very long meeting with my social case worker and my job coach, they have been helping me find a job now for over a year and my temp job did not work out cause of my mental issues of anxiety and depression, they suggested that it may be time that I file for social security. SO I just spent some time doing research on the net as to where I would find the form for me to fill out online or to print up to mail. PLEASE I need your advice cause I am not having any luck.
Could someone here provide me a link (I live in the US).

Im surprised they didnt give you the number and address and website for your local/nearest social security office. you can find those in your phone book under government agencies.

Go to the disabilities link
It's a lengthy application and you will need doctors to fill out certain portions to verify your disability have a record of ALL of yor hospitilaizatuons at least 10 years back an md what for current and past treatments, diagnosis all doctors seen tests run ie mris or cat cans etc the more you have the better included any surgeries reason for time off work all awnser thourghouly and truthfully hood luck

I would go into your local social security office and get the forms and go over them/talk to someone to make sure you understand what your next step is:
